Village Health Partners, a primary care practice in Plano, Texas, and Cigna have launched a collaborative accountable care initiative.
The CAC initiative aims to expand patient access to care and achieve the triple aim of accountable care organizations.
Village Health Partners will use clinical care coordinators to help patients with chronic conditions schedule appointments, provide health education and generally work through the healthcare system. The care coordinators are employed by Village Health and are aligned with Cigna case managers to ensure collaboration.
The initiative was effective Jan. 1, 2013.
