Clearwater, Fla.-based BayCare Health System's physician group, BayCare Physician Partners, and Cigna have launched an accountable care initiative, effective Jan 1.
The initiative expands patient access to care, improves care coordination and strives to achieve the triple aim of accountable care organizations.
As part of the initiative, Cigna provides patient-specific data to BayCare nurse care coordinators to help best treat patients with chronic conditions. About 13,000 patients will be covered by the agreement.
This is not the first commercial accountable care agreement BayCare has signed. The system partnered with Aetna in December for a similar program.
