Here is a list of ACOs that have formed since July, starting with the most recent.


1. Montefiore Medical Center Signs ACO Agreement with Empire BlueCross BlueShield
Montefiore Medical Center in New York City signed an agreement to form an ACO with Empire BlueCross BlueShield.

2. Dominican Hospital, Physicians Medical Group and Blue Shield to Launch ACO in California
Blue Shield of California, Dominican Hospital in Santa Cruz County, Calif., and Physicians Medical Group of Santa Cruz County, Calif., announced a new accountable care partnership.

3. Key Physicians, BlueCross to Launch ACO in North Carolina
BlueCross BlueShield of North Carolina and Key Physicians, an independent patient-centered medical home network based in Wake County, N.C., announced they are teaming up to launch an accountable care organization.

4. Methodist Le Bonheur, BlueCross to Launch ACO in Tennessee
Memphis, Tenn.-based Methodist Le Bonheur Healthcare and BlueCross BlueShield of Tennessee agreed to launch a five-year accountable care organization, effective Jan. 1, 2013.

5. Aurora Health Care, Anthem Blue Cross Ink New Health Plan
Milwaukee-based Aurora Health Care and Anthem Blue Cross and Blue Shield in Wisconsin are linking up for a new health plan designed to improve employees' healthcare experiences while offering a price guarantee to employers.

6. Optimus, Healthcare Partners, Horizon Blue Cross of New Jersey Link for New ACO
Optimus Healthcare Partners — a New Jersey accountable care organization founded by Summit-based Vista Health System IPA and Central Jersey Physician Network — partnered with Horizon Blue Cross Blue Shield of New Jersey for a new ACO agreement covering 11 counties. 

7. Dignity health, Vanguard Pursue Medicare ACO in Arizona
San Francisco-based Dignity Health and Nashville, Tenn.-based Vanguard Health Systems announced plans for their Arizona hospitals to form an accountable care organization and file for participation in the Medicare Shared Savings Program.

8. Park Nicollet in Minnesota Forms ACO With Non-Profit Insurer Medica
St. Louis Park, Minn.-based Park Nicollet Health Services teamed up with non-profit payor Medica to establish an accountable care organization option for people who receive insurance coverage from employers purchasing Medica health plans. 

9. Penobscot Health Center Joins Eastern Maine ACO
Penobscot Community Health Care in Bangor, Maine, joined Eastern Maine Health System's accountable care organization.

10. Aurora Health Care, Aetna Form ACO
Milwaukee-based Aurora Health Care and Aetna are forming an accountable care organization that will go into effect in 2013.
