Coral Gables-based Baptist Health South Florida has partnered with Raziel Health to use its remote care services and platform to remotely monitor patients.
The platform will enable Baptist Health South Florida physicians and care teams to remotely monitor patients with complex disease conditions, and those transitioning from a hospital to their homes, according to a Sept. 22 press release.
Baptist Health South Florida will use the technology to monitor patients with heart failure and complex hypertension.
"This innovative technology allows for care beyond the traditional clinical setting and identifies possible health issues for early intervention. We are partnering with Raziel for its unique integration with Cerner and track record of clinical excellence," said Jonathan Fialkow, MD, chief population health officer at Baptist Health South Florida.
