Clearwater, Fla.-based BayCare Health System, BayCare Physician Partners, the system's 1,100-physician clinically integrated network, and Aetna have partnered in a collaborative care agreement that aims to improve care quality and reduce costs.
The quality and efficiency measures of the agreement include:
• Percentage of members who received recommended screenings and preventive care
• Improved management of patients with chronic conditions
• Reduced hospital readmission rates
• Reduced avoidable emergency room visits
The agreement will cover Aetna commercial and Medicare Advantage members. Patients who have received care from a BayCare Physician Partners physician in the last 24 months will be included in the program.
