The Green Mountain Care Board, an independent board designed to oversee the design of a comprehensive healthcare system in Vermont, announced (pdf) the approval of a new healthcare delivery system and payment reform pilot for cancer in the St. Johnsbury, Vt., area.
The three-year pilot aims to improve the quality of care for cancer patients, increase patient experience and satisfaction, reduce avoidable utilization of services and decrease the growth in overall expenditures related to this care. The new delivery system will be based on Vermont's Blueprint for Health, a medical home model centered on primary care.
Under the initiative, Community Health Teams, groups of health professionals created by primary care practices, will coordinate and monitor patients' care. The teams will coordinate services across primary care, cancer care and palliative care. The providers are expected to receive an additional monthly fee from commercial payors and Medicaid for creating care plans.
The pilot is a collaboration between the three organizations that treat the majority of the St. Johnsbury area cancer patients: Northern Counties Health Care, Northeastern Vermont Regional Hospital and Dartmouth-Hitchcock Medical Center's Norris Cotton Cancer Center North, all located in St. Johnsbury.
