Shelbyville, Ind.-based Major Health Partners is collaborating with software company Unite Us to install a tool that will help employees screen patients for social needs and connect them to resources such as housing, food assistance, transportation and employment assistance.
The tool will note when a patient requires social assistance and works to standardize the health system's referral process for social needs. Unite Us software can integrate within Major Health Partners' Meditech EHR, according to an April 12 Major Health Partners news release.
"At MHP, we are fully embracing the effort of becoming a true community health improvement organization — one that can make a meaningful and measurable generational improvement in health and well-being in our region," Stephen Black, MHP's director of behavioral health and health equity, said in the release. "For MHP to be successful — and for our community to thrive — we all have to reach beyond our four walls and build partnerships to harness the power of collective impact."
