Supporting over 2,500 hospitals, ACOs, and physician practices, along with 130,000 post-acute and community care providers across the United States, WellSky facilitates care transitions for more than 17 million patients each year.
A recent report, released by WellSky and drawing on WellSky data, delves into delves into the transformations in care delivery over the last year, the significant impact these transformations have had on patients and providers as they navigate the evolving healthcare landscape, and industry trends. Below, we touch on two of those trends.
The rise of value-based care enablers
As the industry continues its transition to value-based care (VBC), the landscape of VBC enablement organizations is rapidly evolving. These entities collaborate with independent providers and medical groups to facilitate the adoption of value-based models. VBC enablers manage the total cost of care for specific patient populations, such as those with chronic illnesses or conditions like chronic kidney disease, cancer, or dementia, ultimately improving the quality and efficiency of healthcare delivery.
As VBC enablers take on more risk, they need a better way to connect to acute and post-acute care teams and engage more efficiently with their patients to control spending. The data shows that by leveraging WellSky technology and services, providers are better able to influence patient care transitions and improve patient outcomes.
Regulatory shifts
The Centers for Medicare & Medicaid Services is continuing to drive change and innovation — and make good on their goal of ensuring all Medicare beneficiaries are in a value-based care arrangement by 2030. It's doing this with new models of care, such as the Transforming Episode Accountability Model (TEAM), ACO Primary Care Flex (ACO PC Flex), and Making Care Primary (MCP). With some CMS programs winding down in 2025, such as Bundled Payments for Care Improvement (BPCI) Advanced, these more recent initiatives like TEAM are poised to transform the healthcare landscape by enhancing care coordination, improving patient outcomes, and addressing health disparities while underscoring the importance of a connected continuum of care and the need for collaboration among all stakeholders.
