Livonia, Mich.-based Trinity Health's national home healthcare agency, Trinity Health At Home, will utilize Plano, Texas-based Vivify Health's technology to bring virtual care to home health patients.
Trinity Health At Home's Home Care Connect program will use Vivify Health's technology to allow patients to virtually connect with providers. Patients will be equipped with a tablet with which they can monitor and report on their health.
The go-live will take place later this month in Trinity Health's southeastern Michigan communities. It will later expand to other Trinity Health communities in California, Illinois, Indiana, Iowa, Maryland, Michigan and Ohio.
