ISMP updates list of look-alike drug names

The Institute for Safe Medical Practices has revised its list of look-alike drug names that are often confused, adding more than 80 drug pairs to the resource. 

The list now includes drug names that use mixed-case letters. Healthcare organizations can use the list to determine which medications require special safeguards to reduce the risk of errors. 

Strategies include using both brand and generic names on labels and configuring computer systems to require a minimum of five letters when searching for a drug name.
