PinnacleHealth, Capital BlueCross Form Accountable Care Arrangement

Harrisburg, Pa.-based PinnacleHealth System has teamed with Pennsylvania insurer Capital BlueCross to form an accountable care arrangement, an effort to enhance care coordination and patient satisfaction while reducing healthcare costs.

Capital Blue will work with Pinnacle physicians to drive improvements in the delivery system and will provide nursing resources, access to technology and critical analytics to the physicians. The insurer and health system plan to use population-based clinical data to deliver personalized medical care to the more than 25,000 patients covered under the new accountable care arrangement.

In 2010, Capital BlueCross and PinnacleHealth implemented a patient-centered medical home pilot in Pennsylvania.
