New Medical Home Network Will Coordinate Care for Chicago Medicaid Patients

Illinois' Cook County Health and Hospitals System, five private hospitals and dozens of clinics and physician practices will launch the Medical Home Network this summer, which will cover more than 162,000 residents on Chicago's Southwest Side, according to a Chicago Tribune report.

Hospital partners in the venture include the following Chicago-area facilities:
• Cook County's Stroger Hospital, Provident Hospital and Oak Forest (Ill.) Hospital
• Rush University Medical Center
• Mt. Sinai Medical Center
• Holy Cross Hospital
• Saint Anthony Hospital
• La Rabid Children's Hospital

The venture has been called one of the most ambitious attempts in the country to restructure care delivery to vulnerable or impoverished neighborhoods and Medicaid patients. The network has already established working relationships between dozens of medical providers that have traditionally held competitive, rather than cooperative, relationships.

Medical Home Network plans manage care transitions by linking hospitals and medical clinics through an Internet portal, according to the report. When a Medicaid patient is discharged from the hospital, participating clinics will receive alerts so they can offer follow-up care within a week.

Funding for Medical Home Network has been provided by Chicago's Comer Science and Education Foundation. Eventually, the network plans to extend to Chicago's Southeast Side.
