3 Delaware Hospitals Collaborate to Tackle Diabetes, Cancer

Dover, Del.-based Bayhealth Medical Center announced a new collaboration with Beebe Medical Center in Lewes, Del., and Seaford, Del.-based Nanticoke Health Services to enhance health in the community.

The hospitals created an initiative called Healthier Sussex County with the goal of making Sussex County one of the healthiest counties in the nation. The initiative aims to identify and address health disparities, improve health education and encourage healthy behaviors. The hospitals' first focus will be diabetes and mammography.

The Diabetes Initiative will involve a one-year pilot project with 10 primary care practices. The project aims to increase the percentage of patients with diabetes who have completed HgbA1C testing in the last year from 89 percent to 92 percent among Medicare patients and from 70 percent to 75 percent among Medicaid patients. The test shows average blood sugar over the past three months and can indicate the quality of blood sugar control, according to the release. A second goal is to increase participation in diabetes education and self management programs from 50 percent to 55 percent.

Under the Mammography Initiative, the hospitals in Healthier Sussex County will work with the Komen Foundation, Delaware Breast Cancer Coalition, Delaware Public Health and the American Cancer Society to increase breast health education and mammography screening in the community.
