ONC Pilots Test Interim Standard for Health Information Exchange in Care Transitions

The Standards and Interoperability Framework, part of ONC, is beginning pilot projects to test an interim standard for exchanging core summary data when patients move to different care settings, according to a Government Health IT report.

The pilots, which are part of the Transitions of Care initiative, include 10 health IT and health information exchange vendors and aim to show the ability of a transitions-of-care standard to support stage 2 of meaningful use.
