MDwise, an Indiana non-profit health care program, alerted members that 2,700 personal records were leaked to the Internet, according to an IndyChannel.com report.
The leak resulted from attempts to upgrade MDwise's customer record computer system. The leaked health records contained members' names, addresses and Medicaid numbers.
