The ECRI Institute recently analyzed electronic health record issues of a small sample of hospitals from reports collected between 2009 and 2011, and it found several EHR issues that contribute to adverse events.
Some of the most common EHR adverse events included the following:
• Delay/failure of entering the physician order
• Discrepancy between the electronic medical record and the paper chart
• Delay/failure to act on charted order
• Wrong order entered/order entered in error
• Discrepancy among EMR views or EMR and linked systems
• Wrong patient/patient identification problem
The report noted that some of the problems are unique to health IT, such as new technology learning curves, alarm fatigue and data corruption that can lead to a delay in diagnosis.
