Fort Myers, Fla.-based Lee Health is planning to launch an acute hospital at-home program and expand its remote patient monitoring program.
The health system has partnered with Biofourmis, a technology company offering platforms that support in-home acute care delivery, to support the programs, according to a Nov. 18 news release.
Under the new partnership, Biofourmis will support Lee Health's entire RPM service line and support the launch of a new hospital-at-home program.
Lee Health is a five-hospital system in Southwest Florida. With an average daily census of more than 700 patients, its RPM program is one of the largest in the country and has driven a 50% reduction in 30-day readmission rates for all clinical pathways, according to the release.
CMS approved Lee Health's at-home model in May 2023 at two of its two largest hospitals: Lee Memorial and HealthPark Medical Center.
