Horizon Blue Cross Blue Shield of New Jersey launched an accountable care organization Jan. 1 with LHS Health Network, a network of physician practices, hospitals and other healthcare providers affiliated with Lourdes Health System in Camden, N.J.
The ACO covers more than 17,000 Horizon BCBSNJ members and includes 90 primary care physicians.
Through the partnership, Horizon BCBSNJ provides care coordination payments to providers, and the providers take responsibility for improving outcomes and experience and lowering costs. Additionally, if the providers meet goals for health improvement, patient satisfaction and costs, they can share in achieved savings.
"We are extremely pleased to be developing this innovative model of service with Horizon, who share our vision for providing high quality, high value care to the southern New Jersey community," Alexander Hatala, president and CEO of Lourdes Health System and the LHS Health Network, said in the release.
