Humana and Milwaukee-based Aurora Health Care announced an accountable care agreement for Humana Medicare Advantage patients, effective Jan. 1, 2015.
Under the agreement, Humana Medicare Advantage patients seeking care with Aurora's network of 1,500 physicians will receive care structured around Humana's Accountable Care Continuum. The continuum strives to improve medical outcomes, patient experience and cost through value-based, highly integrated care.
The organizations are seeking to improve medical record infrastructure and collaborate disease management prevention to help improve care, according to a statement.
Aurora Health Care will offer Humana's Accountable Care Continuum at its 15 hospitals, 159 outpatient facilities and 70 retail pharmacies throughout eastern Wisconsin and northern Illinois.
