Detroit-based Henry Ford Health has created a new company called Populance to support hospitals, physicians and health plans in advancing population health efforts.
Four details:
- The subsidiary will leverage Henry Ford's expertise in managing patients with complex conditions to support other health systems, insurers and physicians in improving patient outcomes while lowering costs of care. The new company's launch comes as more insurers move to value-based models, which base payments on quality and outcomes versus the services provided.
- Populance will offer a variety of services, including care management, patient monitoring and digital outreach tools. Case management is a core component of the company's solutions, according to Henry Ford Health. Case managers will work with partners to ensure their patients understand and stick to care plans, and connect them to appropriate resources to support their health and well-being.
- Christopher Stanley, MD, will serve as president of Populance. He is the former chief population health officer at Sutter Health in Sacramento, Calif., and has spent more than 30 years in healthcare, working across health systems, health plans and physician groups. The company is headquartered in Troy, Mich., and employs 150 team members.
- Henry Ford Health said Populance is an extension of the health system's leadership and success in advancing population health regionally. The health system and its nonprofit health plan, Health Alliance Plan, have created dozens of programs over the past two decades meant to support patients in managing chronic conditions.
"Because we know this approach to population health management works — for our patients, our members and our physicians — we want to make these services available to other physicians, health systems and health plans to create healthier, more equitable outcomes in all the communities we serve," Robin Damschroder, chief financial officer and president of value-based enterprise at Henry Ford Health, said in a news release.
