Denver-based SCL Health System, an 11-hospital system, and Scottsdale, Ariz.-based Univita Health, a home care management company, announced the creation of a joint venture.
The joint venture is designed to create a smooth transition for patients from inpatient acute care to outpatient, home-based care. Through this partnership, the organizations aim to reduce hospital length of stay, reduce unnecessary hospital readmissions and improve patient experience, according to the news release.
SCLHS and Univita are launching this integrated acute/post-acute care delivery model at three SCLHS hospitals: Exempla Lutheran Medical Center in Wheat Ridge, Colo., Exempla Saint Joseph Hospital in Denver and Exempla Good Samaritan Medical Center in Lafayette, Colo.
