The University of Kansas in Lawrence announced a collaboration with Lawrence (Kan.) Memorial Hospital to reduce readmissions of older adults.
The Office of Aging and Long Term Care at KU's School of Social Welfare and Lawrence Memorial Hospital developed the Peer Support and Wellness for Older Adults Program. Under the program, community volunteers aged 55 and older will be matched with individuals with chronic disease at high risk for readmission after hospital discharge.
The volunteers will receive training to help individuals identify their strengths and resources in the community and determine health goals. The pairs will then develop and implement an action plan to meet defined goals. Volunteers will meet with their matched individuals twice a week for 10 weeks.
