In support of National Patient Safety Awareness Week, AHRQ's new toolkit aims to help hospitals improve their medication reconciliation processes to reduce adverse drug events. The Medications at Transitions and Clinical Handoffs Toolkit provides step-by-step instructions on how to improve medication reconciliation, from planning — including how to get leadership support — to pilot testing, implementation and evaluation.
