Canada's largest cardiac center — the Montreal Heart Institute — has chosen SeamlessMD, an integrated care solutions provider, for patient engagement and care management technology.
SeamlessMD's technology platform allows patients to connect with their care team and provides patients with daily reminders leading up to surgery, check-ins to monitor progress after surgery, personalized recovery recommendations and educational multimedia resources to improve home recovery.
"SeamlessMD helps our hospital accomplish key goals: greater innovation, shorter recovery times and better patient experience," said Louis Perrault, MD, chief of cardiac surgery at MHI, according to an emailed release. "Recognizing the transition to value-based care for cardiac surgery, such as bundled payments, SeamlessMD will be a key partner of ours for improving the patient experience and clinical outcomes for cardiac surgery."
