Boston Children's Hospital is partnering with Medumo, a patient navigation platform provider, to improve patient access and experience during their care journeys.
Per the partnership, Medumo's CareTour platform will incorporate DisCo, a post-discharge application developed by Boston Children's Hospital to improve follow-up care by enabling patient-specialist communication. The CareTour solution sends notifications and instructions to patients to ensure they have all the information they need prior to surgeries and appointments. It helps predict which patients are at higher risk of being no-show or ill-prepared so caregivers can intervene early.
Together, the two solutions aim to offer patients support through their journey. Boston Children's Hospital and Medumo will co-deploy the combined technology across three departments within the hospital: gastroenterology, the adolescent medicine long-acting reversible contraception program and bariatric surgery.
