WVU Medicine partnership aims to address social determinants of health

Morgantown-based West Virginia University Medicine partnered with social care network Findhelp to address social determinants of health such as transportation, food, and shelter, according to an Oct. 4 report from WV News.

The Findhelp network allows users to access 605,700 distinct program locations in all 50 U.S. states, territories, and Puerto Rico. According to the report, users can search by ZIP code to access free or reduced-cost services. 

WVU Medicine Population Health Director Chris McCormick said chronic diseases can all be negatively impacted by socioeconomic factors.

"If we can help connect our patients to things like shelter, transportation to medical appointments, and food, we can help them better manage their chronic conditions and live fuller, healthier lives," said Mr. McCormick. 

Findhelp will be available to WVU Medicine patients through the online portal MyWVUChart, and patients can search anonymously for assistance.
