Austin, Texas-based Seton Healthcare Family and geriatric care specialty group ProperCare, also in Austin, are partnering on a pilot program to equip aging patients and their families with the support and resources necessary to recuperate at home post discharge.
ProperCare will utilize Seton Healthcare's provider network to train a team of care professionals to aid patients during the transition and to help them take full advantage of commonly underused Medicare benefits and community resources at no cost to them.
The aim of the partnership is to provide services to 400 patients within six months at three participating Seton Healthcare facilities: Austin, Texas-based Seton Northwest Hospital, Kyle, Texas-based Seton Medical Center Hays and Round Rock, Texas-based Seton Medical Center Williamson.
"We're looking forward to working with Seton to identify the most vulnerable of their elderly patient population and provide care management services that help them recover to their full capacity at home," said Bill Wagenbaur, executive vice president and director of post-acute transitions at ProperCare. "Many times, elderly patients don’t have the support they need. We serve as surrogate family members to advocate for them and ensure they have the best possible care plan in place."
