Kennett Square, Pa.-based Genesis HealthCare and Louisville, Ky.-based Kindred Healthcare declared their intent to form a strategic clinical collaboration to develop and implement healthcare initiatives to improve quality of care, patient safety and efficiency.
Under the terms of the agreement, Genesis and Kindred aim to promote physician collaboration and integration, support the development of post acute care networks, identify ways to improve care and outcomes for specific patient populations, better understand population health and develop processes, guidelines and tools for post acute care discharge planning.
