The Health Resources and Services Administration Geriatrics Workforce Enhancement Program awarded a three-year, $2.55 million grant to New York University College of Nursing's Hartford Institute for Geriatric Nursing, to build the primary care workforce in clinical and community settings, according to NYU.
Tara Cortes, PhD, RN, professor of geriatric nursing at NYU College of Nursing and director of the Hartford Institute for Geriatric Nursing, was awarded the grant. Dr. Cortes and her team will partner with Montefiore Health System in the Bronx, New York City, and the Bronx-based nonprofit Regional Aid for Interim Needs to implement the initiative.
The project, which will be implemented in five Montefiore Medical Group patient-centered medical homes and several RAIN sites, is called the "Community/Clinical Primary Care of Older Adults Program." It is a multidisciplinary educational intervention, aiming to accomplish the following objectives.
- Provide information and resources to primary care professionals to provide age-sensitive, team-based, family-centered, evidence-based care for seniors.
- Give providers and community organizations the tools to coordinate healthcare and disease-management on a community level.
- Use community resources to educate seniors, families and caregivers.
- Establish a comprehensive Alzheimer's Disease and Related Dementias program.
