Niagara Falls (N.Y.) Memorial Medical Center and Eastern Niagara Health System, in Newfane and Lockport, N.Y., has joined with mental health services provider Horizon Health Services as co-leaders of a potential "health home" for Medicaid patients in the Niagara area, according to a Buffalo News report.
New York State is expected to announce providers selected for its Health Homes program later this month. The program is intended to better coordinate care for Medicaid patients and reduce the costs of caring for this population.
The Niagara Health Home would include 39 primary care providers and additional specialty providers. The group is expected to initially care for 2,500 Medicaid enrollees with at least two chronic conditions, but could grow to oversee care for up to 6,000.
