CPC+ is CMS' largest primary care transformation initiative to date. It is designed to give physicians the flexibility to deliver care beyond face-to-face encounters and it is a path for participation in the Advanced Alternative Payment Model under MACRA, the Medicare Access and CHIP Reauthorization Act.
