Leavitt Partners, Forward Health to roll out CPC+ support for physicians

A consultancy and a software company are teaming up to offer physician groups and payers support as they navigate CMS' Comprehensive Primary Care Plus payment initiative.

Salt Lake City-based healthcare intelligence firm Leavitt Partners and Forward Health Group, a population health management software company based in Madison, Wis., formed the alliance to help physician practices eliminate some of the hassle associated with starting a new program. One benefit of the LP-Forward Health alliance is physician practices that plan to apply for Track 2 of CPC+ will need a "Letter of Support" from an approved health IT vendor — which Forward Health Group qualifies as.

CPC+ is CMS' largest primary care transformation initiative to date. It is designed to give physicians the flexibility to deliver care beyond face-to-face encounters and it is a path for participation in the Advanced Alternative Payment Model under MACRA, the Medicare Access and CHIP Reauthorization Act.
