Irvine, Calif.-based ConcertoHealth opened two new care centers in the Seattle area to better serve the state's dual-eligible Medicare-Medicaid population.
Complex Medicare and Medicaid patients total more than 180,000 residents of Washington state. By adding two new care centers, ConcertoHealth effectively triples its presence in Washington to provide better care to these patients.

"Our care centers integrate clinical care with social services to ensure patients and their families receive the individualized support they need to improve health outcomes. We're an organization that is changing the way healthcare is practiced for complex medical needs patients in Washington state and nationwide by providing medical, social and logistical support to our patients, whenever and wherever they need it."
The two new centers will be located in Lynnwood and Lakewood, Wash. Each center will provide 24/7 care coordination; chronic disease management and prescription assistance; access to physicians, nurses, social workers, medical assistants and pharmacy staff; access to community and social services; and health education resources.
