CMS opens application period for primary care model

CMS is now accepting applications for its new primary care medical home model: Comprehensive Primary Care Plus.

The five-year CPC+ program is slated to begin January 2017 for an estimated 5,000 primary care practices around the country. CMS has also said the model will likely qualify for as an advanced Alternative Payment Model under MACRA, the Medicare Access and CHIP Reauthorization Act. This means providers participating in the model could be eligible for an incentive payment as part of the new Medicare payment adjustment model.

CMS is offering the CPC+ model in 14 regions. The program operates as a public-private partnership between Medicare, Medicaid, private payers and primary care practices.

"As a key part of CPC+, CMS and partner payers are committed to supporting primary care practices of all sizes, including small, independent, and rural practices," Patrick Conway, MD, CMS deputy administrator and CMO, said in a statement. "We see CPC+ as the future of primary care in the U.S. and are pleased to partner with payers across the country that are aligned in this mission to transform our healthcare system. This model allows primary care practices to focus on what they care about most — serving their patients' needs when and how they choose."

The following states will participate in full:

  • Arkansas
  • Colorado
  • Hawaii
  • Michigan
  • Montana
  • New Jersey
  • Oklahoma
  • Oregon
  • Rhode Island
  • Tennessee

The CPC+ program will additionally be offered in the following regions:

  • Greater Kansas City Region (Kansas and Missouri)
  • North Hudson-Capital Region (New York)
  • Statewide and Northern Kentucky Region (Ohio)
  • Greater Philadelphia Region (Pennsylvania)
