Cigna and New Haven (Conn.) Community Medical Group, a network of 216 primary care physicians and 367 specialists, have launched a collaborative accountable care initiative.
The collaborative accountable care approach aims to achieve the same goals as accountable care organizations.
The program will include around 18,000 patients covered by a Cigna health plan who receive care from New Haven Community Medical Group's physicians.
Care coordinators, employed by the medical group and aligned with Cigna case managers, will help patients with chronic conditions navigate the healthcare system.
Cigna will compensate the medical group for its medical and care coordination services, and the group will be rewarded through a pay-for-performance structure if it meets quality and cost targets.
