Cigna has launched an accountable care initiative with the Weill Cornell Physician Organization in New York, according to a joint news release.
The initiative is essentially an accountable care organization since it shares the model's population health goals. It's the first patient-centered accountable care initiative in New York City involving a health plan and physician organization.
Individuals covered by a Cigna health plan can receive care from among 71 Weill Cornell primary care physicians, and care coordinators will also help patients navigate the continuum. Patients will continue to see their current physician and those most likely to see the immediate benefits of the program are those who need help managing chronic illnesses.
With this development, Cigna is now engaged in 17 patient-centered health initiatives in 15 states.
