Greenville, S.C.-based Bon Secours St. Francis Health System's medical group and BlueCross BlueShield of South Carolina have launched a patient-centered medical home for patients with diabetes, hypertension or congestive heart failure.
Primary care physicians in the Bon Secours Medical Group will be tasked with coordinating patient care, including scheduling checkups, tests and making referrals to other providers, as well as following-up with patients after emergency services and hospitalizations.
The BCBS-Bon Secours medical home will care for up to 2,900 eligible patients. BCBS in South Carolina has implemented PCMH pilots in several other cities across the state.
