The Blue Cross and Blue Shield Association highlighted its patient-centered medical home collaborations with providers across the nation at a Capitol Hill briefing on Monday.
BCBS says it has helped established PCMH initiatives across 39 states, the District of Columbia and Puerto Rico that cover more than four million Blue Cross members.
At the briefing, BCBS highlighted two initiatives: one based in North Carolina, an agreement between UNC Health Care and Blue Cross, and another in New Jersey, between Horizon Healthcare Innovations and both the Duke University School of Nursing and Rutgers College of Nursing. The collaboration trains New Jersey nurses to serve as PCMH-based care coordinators.
