The collaboration between a health system, an insurer and a technology firm is the first of its kind in the Philadelphia area, according to the press release. The three organizations will work to coordinate patient care, lower overall care costs and improve patient and physician satisfaction.
Abington Health and IBC will pool their patient data — including claims, preauthorizations, hospitalizations and clinical lab results — and Lumeris will aggregate and interpret the data.
Abington Health's physicians will be reimbursed under IBC's Integrated Performance Incentive Plan, which rewards physicians and hospitals for improving quality and care coordination.
