Adirondack Community Physicians and St. Elizabeth Medical Group announced Tuesday they will merge as the Mohawk Valley Health System Medical Group, one year after their health systems, both based in Utica, N.Y., affiliated to form the Mohawk Valley Health System.
Together the new medical group includes 32 locations and 130 providers distributed across 19 primary care offices, a children's health center, a women's health center, a breast care center, two urgent care centers and general, orthopedic and neurological surgery practices, according to the report.
The St. Elizabeth Medical Group practices will become patient-centered medical homes, similar to the model used by the Adirondack Community practices.
