Hartford (Conn.) HealthCare has created a new leadership structure, wherein the roles of president and CEO have been separated.

Here are five things to know:

1. Hartford HealthCare's current President and CEO Elliot Joseph will remain as the system's CEO.

2. Jeffrey Flaks will serve as president and keep his title of COO. He currently serves as executive vice president and COO of the system.

3. As president, Mr. Flaks will have overall responsibility for the direction of clinical operations, such as the successful implementation of the Epic electronic health record system.

4. In addition, Tracy Church, previously the health system's chief human resources officer, will be named chief administrative officer.  David Whitehead, Hartford's chief strategy and transformation officer, will have additional responsibilities.

5. The changes will take place Oct. 1.
