The University of Kansas Hospital in Kansas City plans to implement Cerner's HealtheIntent population health management platform in its Kansas Heart and Stroke Collaborative.
The collaborative focuses on improving heart health and stroke outcomes in rural Kansas communities. It will use HealtheCare, Cerner's community care management solution, to conduct assessments and develop interventions when necessary.
"Rural communities have many patients with chronic conditions, such as heart disease and stroke, where care coordination and patient education are important to improve outcomes," said Robert Moser, MD, director of the Kansas Heart and Stroke Collaborative. "Leveraging HealtheCare will enable our Heart and Stroke Collaborative partners and local health coaches to engage at-risk patients at the right time, providing the optimal care and resources they need to assist in their treatment."
