Tabula Rasa HealthCare, a Moorestown, N.J.-based healthcare technology company, partnered with Tandigm Health, a West Conshohocken, Penn.-based a healthcare company that supports primary care physicians, to improve medication regimen assessments for high-risk patients transitioning to home care.
Tandigm will utilize TRHC's medication risk mitigation platform, called MedWise Advisor, to assess medication regimens of patients recently discharged from the hospital to reduce adverse drug events and hospital readmissions.
