The 2011 Survey of Patient Privacy Breaches found that in the past 12 months, 71 percent of respondents had a breach of PHI, more than 38 percent reported two or more types of breaches and 20 percent had three or more types of breaches.
Snooping into medical records of employees was the most commonly reported type of breach at 35 percent, followed by snooping into medical records of friends and family at 27 percent and loss or theft of physical records at 25 percent
