Princeton HealthCare System in Plainsboro, N.J., has selected an integrated population health platform from e-MDs and Lightbeam Health Solutions.
The platform is used by approximately 40 ACOs and draws patient data from e-MDs' EHR platform. The system then provides an overview of cost and utilization from payers to providers and uses Lightbeam's analysis and cloud solutions to make the data understandable in order to determine areas for improvement.
Lightbeam provides population health software that integrates with e-MDs' existing EHR and practice management software. It also provides a risk stratification tool to prioritize the patients most in need of care, according to Lightbeam's website.
"The new tool will give our clinicians the ability to address questions across the disease spectrum, keep track of attributed patient populations, track risk stratification and ensure quality performance," said Tobe Fisch, MD, a practicing physician and director of practice innovation for Princeton Medicine, the employed physician practice of Princeton HealthCare System. "We expect the e-MDs and Lightbeam solution will help facilitate better team-based care by providing an immediate overview for identifying gaps in care for individual patients."
