Charlotte, N.C.-based Premier has launched CommunityFocus, a cloud-based population health management solution.
CommunityFocus was developed in partnership with University of North Carolina at Charlotte. It seeks to help health systems acquire and manage community-level health data and also develop interventions to improve population health management.
Minneapolis-based Fairview Health Services and Wayne Memorial Hospital in Goldsboro, N.C., have signed on to be early users of CommunityFocus.
