Norwalk Hospital receives award for data-driven care delivery

Norwalk (Conn.) Hospital was recognized with the John D. Thompson Award for Excellence in the Delivery of Healthcare Through the Use of Data at the Connecticut Hospital Association's annual meeting this month.

The hospital earned the award due to the success of its Total Joint Replacement Destination Center of Excellence. The center was established in 2012 and uses data to track and advance improvements in length of stay, discharge home and patient satisfaction for joint replacement surgery patients.

The center achieved those improvements by developing standardized care pathways; providing pre-op education for patients and caregivers; creating a cohorted care unit comprised only of specialized team members; advocating for non-opioid pain management options and agents; establishing nurse-driven early and frequent mobility; and integrating an assistive patient navigator. Data regarding these and other measures of patient care is consistently tracked and regularly validated to identify trends.

"Norwalk Hospital demonstrated that its patient- and family-centered multidisciplinary approach, along with applying best practices and reducing variation, led to better outcomes and greater patient satisfaction," Jennifer Jackson, CEO of the CHA, said.
