Presbyterian Anesthesia Associates in Charlotte, N.C., disclosed that a hacker gained access to the personal information of 9,988 people by breaking through a security loophole on the practice's website, according to a Charlotte Observer report.
The information exposed included names, contact information, dates of birth and credit card numbers. It did not include medical information.
The practice notified the FBI, which has launched an investigation. The practice has also hired an identity theft firm to provide free monitoring for those affected, according to the report.
