The Indiana Health Information Exchange Quality Health First Program has enrolled more than 1,500 physicians to improve the management of medical conditions through early interventions and follow-up care, according to an IHIE news release.
Patients needing these and other interventions are flagged in reports to their primary care physician, enabling the physician to make the best possible care decisions before, during and after patient visits. The reports are based on clinical information and claims information to provide the most up-to-date, robust and relevant information available. The program allows physicians to review and verify the data and to reconcile missing or inaccurate data.
Participation is open to any primary care practice in Indiana, regardless of its size or number of physicians, and is not restricted to practices that do not use electronic health records. The program provides a structure for health plans to reimburse physicians based on improvements in patient outcomes.
