Long Beach (Calif.) Memorial Medical Center has notified 2,864 patients of a possible breach of their private information, according to a Press Telegram report.
Potentially compromised information includes patients' name, gender, date of birth, address, phone number, insurance information and reason for admission, according to the report.
Affected patients have been offered a free year of a credit monitoring service, according to the report.
