Humana has announced the launch of a telehealth pilot in Ohio for congestive heart failure patients.
Through a partnership with telehealth company AMC Health, Humana will provide in-home monitoring services and support to 450 CHF patients to help improve outcomes. Through the program, patients' monitored data will be compared to nationally accepted parameters, and patients needing intervention will be connected with nurses on the telephone to discuss symptom management and determine if a physician visit is needed.
